Schedule of Fees

The session fees are set to ensure that the prices for my services are inclusive of taxes, superannuation, leave provisions, continuing professional development expenses, and other general business administration and operating costs where applicable.

Session fee’s are dependant on your applicable funding body or referral service. I will provide you with ample notice should the scheduled fees change. The scheduled fees will be in accordance with the National Consumer Price Index.

Cancellation Policy

If You need to cancel a scheduled service, you will need to provide me with a minimum of 24 hours’ notice prior to the commencement of the scheduled service.

If you cancel after this time, it is my right to charge a cancellation fee.

Confidentiality and Consent

In order to provide a service to you, we need to collect and record personal information. This includes information about what is discussed and what happens during your sessions. All interactions which take place in the setting of therapy are considered confidential. This includes requests by telephone, all interactions with AM Therapy staff, any scheduling or appointment notes, all session content records and any progress notes that may be taken during your sessions. The business will not even verify that you are a client to external parties. Any contact between the business and external parties will need to have written permission by way of completing the Consent Form. Your confidentiality and security of your information is taken seriously.

Limits to this Agreement

All personal information gathered by the therapist during sessions and via correspondence will remain confidential except when:

  • If you give your permission to share information with another person or organisation.

  • If we are required to disclose information by law, such as a court subpoena.

  • If we are worried about your safety, or the safety to someone else, and a report needs to be made to relevant authorities (ambulance or police).

  • We may be legally required to report disclosures of recent and historical sexual offending against a child to police.

  • If we are concerned that a child is being harmed or is at risk of harm, a report will be made to The Department of Children, Youth Justice and Multicultural Affairs.

  • If you access services funded by Medicare, we are obligated to provide reports to the referrer (e.g. your GP) including information about assessments and treatment progress. If your services are funded by Medicare or another funding source, we need to advise them about your engagement with us for billing purposes.
